Elder Care Navigator
When an older adult’s care needs change, families often feel overwhelmed—by options, timelines, medical information, facility processes, and the emotional weight of making the “right” decision. The Elder Care Navigator is the steady guide through that complexity.
In this role, you will serve as an advocate and experienced elder care professional on the legal team, working closely with key staff to facilitate all aspects of care planning. You’ll act as the liaison between the law firm, care facilities, and the client, helping families understand care options, supporting transitions, and ensuring the client’s voice and dignity remain central throughout the journey.

Essential Duties & Responsibilities
You will partner with attorneys, long-term care planners, and other key staff to facilitate high-quality life care planning services, including:
Facilitate and assure the quality of all aspects of elder law services for care planning clients and their families.
Provide care coordination to guide clients and families through facility placement and/or transfer.
Coach and support families on advocacy with facilities, staff, and the client’s medical team.
Research and maintain up-to-date information on community resources, facilities, and helpful materials to best support clients engaged in life care planning.
Attend Interdisciplinary Team Meetings (ITM) at nursing facilities as needed; document meeting notes and decisions accurately in the client’s file.
Assess clients’ functional levels and develop care plans and recommendations in conjunction with other professionals on the legal team.
Collaborate with marketing staff to promote the law firm and affiliated services across the service area, including (but not limited to): seminars, articles, radio engagements, event speaking, and referral relationship-building.
Represent the firm to facilities and the healthcare community through regular visits, strengthening positive relationships with leadership and staff.
Attend appointments with attorneys and long-term care planners to interact with clients and referral sources.
Make client satisfaction a top priority by helping resolve concerns with urgency and care.
Maintain confidentiality of all client and personnel information in accordance with firm policies.

Qualifications
Required
Bachelor’s degree in Social Work or a related field (or equivalent work experience).
- OR 2–3 years of experience in a professional environment with strong administrative, coordination, or client-facing responsibilities.
Strong organizational skills and ability to manage multiple priorities with consistent follow-through.
Exceptional verbal and written communication skills.
High professionalism, integrity, and commitment to confidentiality.
Proficiency with Microsoft Office Suite and standard office technology.
